Message: ...No matter what you supplied great info, most dont get in as deep as you and offer very little descriptions at times only wanting a straight answer without having to check things or the know how to do it.
Sometimes with difficult problems it is best to leave testing devices or test parts on until you find the problem, then work backwards.
Going back with everything you found, the three wet/dry plugs was a clue now that we can some hind sight. Perhaps with the crack three of the plugs didn't fire and three did, but would you have been testing the right cylinder? Hard to say since you didn't know this may be the underlying cause to your problem, chances are you would have picked a "good" cylinder.
But since you did find some odd readings on the AFM, you did find "a" problem, just not "the" problem, so you did prevent something else from happening.
You learned some great troubleshooting techniques from evryone that helped you and learned a lot about the engine systems, next time it may not be that big of a deal to find out what is going on.

On some of your things from your list:
1) You shouldn't have to pull the radiator to check the
distributor cap and rotor.
Nor should you have to pull the hood, next time tilt it forward by by removing the front kidney grill and releasing the pin on the arm that helps hold the hood up.

2) I'm the kind of guy that likes to jump straight to the heart
of the matter, not piss around with basics.
Always do the basics first, spark, fuel supply then go from there. This was a tougher problem and it would have taken dumb luck to check the cylinder that wasn't firing. And this may have been two problem issue between the AFM and distributor cap.

3) I was IN THERE, in November, when I replaced the timing belt,
and there was NOTHING GODDAMN WRONG!!!
The elongated hole in the dustcover may nt have mattered much in clearance as I have seen cars with no cover and the cap was still tight.
Maybe the when you tightened down the cap it began to crack, then after some driving is when it got worse then caused your problems?
Any chance it got tightened down a little crooked causing the crack?

This was a very odd thing to have happen between the two components, you did a great job, most others would have towed the car into a shop at this point.
